IMPORTANT INFORMATION
Northside Athletics and Fitness (hereinafter “Northside Athletics”) is committed to conducting its recreational programs and activities in a safe manner and holds the safety of participants in high regard. Northside Athletics continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants safety. However, participants and parents/guardians of minors registering for the listed programs/activities must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.

WARNING OF RISK
Recreational activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk of serious injury when participating in any recreational activity. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers and injuries due to inclement weather, slip and falls, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and all other circumstances inherent to indoor and outdoor recreational activities exists. In this regard, it must be recognized that it is impossible for Northside Athletics to guarantee absolute safety.

WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK
Please read this form carefully and be aware that in signing up and participating in our sports camps, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with this program (including transportation services and vehicle operations, when provided). I recognize and acknowledge that there are certain risks of physical injury to participants in these programs, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation.

I hereby release and agree to hold harmless Northside Athletics and its officials, agents, volunteers, and employees, from any and all claims arising out of, resulting from or connected with my or my minor child/ward’s participation in the program offered by Northside Athletics.

INDEMNIFICATION
I hereby agree to indemnify and hold harmless Northside Athletics from and against any and all losses, liabilities, claims, costs, damages and/or expenses whatsoever paid, incurred and/or suffered, including but not limited to, any and all attorney’s fees, costs, damages and/or judgments Northside Athletics incurs in the event that I or my minor child/ward cause any injury, damage and/or harm to Northside Athletics, its officials, agents, volunteers, employees, and/or other participants of the program. My agreement to indemnify and hold Northside Athletics harmless extends to all loss, whether or not involving claims by third parties.

This Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement extends to all acts of negligence by Northside Athletics, its officers, agents, volunteers, and employees, and is intended to be as broad and inclusive as is permitted by the laws of the State of Illinois in which the this program is conducted and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

I have read and fully understand the above important information, warning of risk, assumption of risk and waiver and release of all claims. BY CLICKING BELOW, I CERTIFY THAT I HAVE READ AND VOLUNTARILY AGREE TO THE TERMS OF THIS WAIVER AND RELEASE OF LIABILITY AND INDEMNITY AGREEMENT.

PHOTO/VIDEO RELEASE
I hereby grant Northside Athletics and Fitness LLC permission to use my/my child’s likeness in a photograph, video, or other digital media (“Photo”) in any and all of its publications, including web – based publications, without payment or other consideration. I understand and agree that all photos will become the property of the Northside Athletics and Fitness LLC and will not be returned. I hereby irrevocably authorize the Northside Athletics and Fitness LLC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the Northside Athletics and Fitness LLC from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason or this authorization.

I HAVE READ AND UNDERSTOOD THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY CLICKING BELOW.
